According to police, a concerned citizen called to report an erratic driver on Beal Street around 1:57 p.m. on Friday. The citizen told police that the driver of the other vehicle, Blake, was "all over the road" and had crossed over the center line several times.

While police were responding to call, Blake reportedly crossed over the center line and struck another driver head on in the oncoming lane on Fort Hill Street.

Two off-duty Hull firefighters who happened to be driving by stopped to help.

A Hull Fire EMT removed a 3-year-old Hull girl from the backseat of Blake's car. She was not injured, but was transported to an area hospital as a safety precaution. The child is related to Blake.

The driver of the other vehicle, 67-year-old Charleen Phillips, of Scituate, was seriously injured and transported to South Shore Hospital. Her condition is not known at this time.

Blake admitted to police that she had taken several medications before driving, including Oxycodone, Percocet and Klonopin. She had slurred speech and pin point pupils.

Blake's first OUI offense (alcohol) occurred in 1999.
